Upcoming Person-Centered Planning Learning Institutes

The NYS Department of Health has scheduled more training opportunities around the state on person-centered planning, thinking and practice. Please see information below on upcoming scheduled dates. Please share the attached flyer and information with members of your Local Long Term Councils and other partners. It is not only open to program administrators, providers and direct service staff, but also consumers receiving home and community-based services.

Please note that NY Connects staff providing Options Counseling/Person-Centered Counseling are required to complete the PCC NWD certification training comprised of on-line modules and one day in-person offered through NYSOFA.

As you may be aware, the Department of Health sponsors the Person-Centered Planning Statewide Training Initiative to provide person-centered planning training and learning opportunities for all Home and Community-Based Services (HCBS) providers, advocates, recipients and their natural supports across the State. This initiative has been extended through early 2022, and we would like to share an exciting, upcoming learning opportunity that you may be interested in.

Starting in August 2021, the Department of Health will offer Individual and Organizational Virtual Learning Institutes. Below are short descriptions of these innovative person-centered planning and practice learning opportunities:

  • Individual Learning Institute: A six-week learning experience that engages each participant at a personal level as they learn how to support individuals receiving home and community-based services to have a more full and meaningful life in their community of choice. These include virtual group sessions, online coaching with international experts in person-centered planning and practice, and access to an online community of peers.
  • Organizational Learning Institute: A 10-12-week, intensive change experience for organizations that have had at least 1-2 key staff participate in the Individual Learning Institute. It is the next step for an organization that is committed to supporting all individuals who receive their services to have a more full and meaningful life. These include participation in virtual group sessions, design workshops, and ongoing coaching.
